Form Instructions
1. Print and complete the appropriate Authorization Agreement Form above.
2. Return it to the KCP Parish Office at 112 W. 8th St., Kaukauna WI 54130.

-You designate the amount that you want to contribute.
-You can increase, decrease or STOP the automatic payments anytime you wish by notifying your financial institution.
